Represent patients coming to CHOP from local, regional and national physicians, hospitals, community at:large, and other ancillary agencies by coordinating their care. Must have the drive to work independently to develop patient and provider relationships and achieve targeted goals. Responsible for coordinating patient care services and maintaining interpersonal relationships with the patient, family and referral source, from initial contact to discharge. Also work with the Cardiac Center Outreach Director and Service Line Director to assist in the development of outreach activities for the Center.:/h2: 13;

:Patient Navigator Responsibilities
Coordinate care with referring clinicians and referral patients to simplify access improve communication and enhance the patient/family experience at CHOP.
:Coordinate a plan of care for referral patients and be the person of contact with their referring physician.
:Maintains a relationship with the patient/family throughout the course of their care, from initial contact to the end of their hospitalization, providing the patient and referring physician with updates and pertinent medical information.
:Meet with local and regional referring physicians and their staff at least once every three months to discuss the Cardiac Center and any issues/concerns/suggestions.
:Identify potential referring physicians and develop a strategy to encourage them to become a CHOP referring physician.
:Identify and attend regional and national conferences, completing applications and arranging for ads, inserts, organizing speakers when needed and manage booth presence.
2. Strategic Planning
Develop and Implement strategies to increase utilization of the Cardiac Center services, programs and physicians.
:Identifies and evaluates new markets, to include statistical analysis of potential markets, estimates of growth, identification of client needs, and competitive analysis
:Identify, design and implement new strategies that will increase Cardiac Center program and service utilization. Obtain approval from Service Line and Outreach Director prior to implementation.
:Maintain a thorough knowledge of the market and assist in the development of marketing strategies
:Develop, refine and update referral tools such as fact sheets, educational DVDs and Web Site.
:Work with Outreach Director to develop and implement a survey tool for referring physicians and cardiac families.
:Provide feedback including competitive information as appropriate to Service Line Director, Outreach Director, CCEC and CHOP leadership
:Resource Utilization and Collaboration
Collaborate with all appropriate CHOP departments to strengthen relationships and facilitate the referral process.
:Meet routinely with strategic internal partners : Cardiac Center Programs, Cardiology, Nursing, Cardiac Surgery and Cardiac Anesthesia, CME Office, Marketing, Neonatal Outreach and Center for Fetal Diagnosis to ensure consistent messaging to referral audience and align key program initiatives.
:Marketing and Web Teams : Work with the Web and Marketing Teams to maintain current, accurate information about the Cardiac Center via print or Web. Assists in the development of new content based on the changing needs of the Cardiac Center and our referral base.
: Data Management
Capture, track and develop reporting mechanisms on key indicators
:Develop and maintain physician/referral contact database
:Provide referral demographics (reports) and analysis to support departmental growth objectives
:Establish and maintain ROI metrics in relation to outreach priorities and departmental initiatives
:Other data needs as determined by Service Line and Outreach Directors
