Specific Responsibilities:
• Adheres and understands the Aleph policies and procedures
• Provide staffing for sick leave, vacation and personal time
• Conduct telephone intakes with new and potential clients
• Coordinate and schedule aides and clients that match the care plan
• Enters scheduling data, client information, communications with client into
database
• Contacts aides and clients when there are changes in the schedule
• Communicate with office staff when clients are discontinuing services and/or
pausing services due to hospitalization
• Discerns client services required as outlined in their agreement, urgent
requests and care plans.
• Provide information to the nurses about the clients and coordinate scheduling
assessments
• Follow up with the clients regarding service delivery within 24-48 hours
• Assist with screening and interviewing potential CHHA’s and caregivers
• Performs general office duties such as filing, organizing, creating folders for
clients/aides
• Troubleshoot and problem solve with aides and client needs. Making sure to be a
matchmaker between aides and clients
• Provide prompt and attentive customer service
• Attend regular employee meetings and trainings
• On call to coordinate clients and home health aides schedules on weekends and
weeknights

Qualifications:

• Associates Degree/GED
• Prior scheduling experience in home care is needed
• Strong computer skills including, but not limited to, Outlook, MS Excel, MS Word
and a Scheduling database
• Ability to deal effectively with the high demands of the fast pace needs in
homecare
• Experience in problem solving
• Basic knowledge in medical terminology
• Strong interpersonal skills, organizational and communication skills
• Must be available to be on call
